To unlock H-scenes for a location, you need to view at least one H-scene in each location. It's easier if you create a male or female character with Being good at persuasion Feature. Then, I play the waiting game and mash that skip time button. (eg: Remove outpatient and doctor and just keep the patient) Add two women of each voice type into the area.

(such as you have to literally wait for them to leave the area before interacting) Maybe I need to make them more social, but like. Like, I can't actually walk around and purposefully interact with other characters? Another thing I noticed, mostly because I no speaky Japanese, but there doesn't seem a way to have different career characters interact. I would be disappointed, but I never had expectations to begin with! RG took 2 steps back from games Ai Girl and Kk.
